The Home Health LPN serves as the primary point of clinical oversight, ensuring care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals, while supporting successful outcomes and transitions across the continuum of care. In this role, the LPN functions as both a clinician and a case manager—conducting holistic assessments, developing and managing individualized plans of care, coordinating services across the interdisciplinary team, and monitoring patient progress throughout the episode of care.
