Strong understanding of patient intake, scheduling workflows, and care coordination* Proficiency in EMR systems (AlayaCare and Kinnser/WellSky preferred) and ability to manage real-time documentation* Excellent critical thinking, communication, and organizational skills* Ability to manage multiple patients, clinicians, and priorities simultaneously in a fast-paced environment* Valid Florida driver's license and reliable transportationPreferred Qualifications* Experience in acute care, case management, hospital discharge planning, clinical coordinator/scheduler or transitional care management* Background in multi-disciplinary coordination and utilization management* Familiarity with Smartsheets or scheduling optimization tools . Key Responsibilities* Partner closely with Scheduling Coordinators to manage daily clinician schedules, routing, and coverage needs* Serve as a clinical liaison between hospital partners, Virtual Nurse/Command Center (Cleveland Clinic), and internal care teams* Coordinate patient admissions, care transitions, and real-time service adjustments* Maintain accurate, real-time documentation and workflows within EMR systems (AlayaCare and Kinnser/WellSky preferred)* Support clinical oversight, staff guidance, and workflow efficiency across the care team* Participate in quality assurance and performance improvement initiatives .
