Impresiv Health is seeking a Nurse Practitioner to join an interdisciplinary care team providing comprehensive medical care and chronic disease management to a focused panel of patients living with chronic kidney disease (CKD) and other complex conditions. The Nurse Practitioner will work closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person.
