Provides service to patients in acute, short, and long-term care settings • Assists physicians in assessing patients and treating injuries and ailments • Obtains a thorough medical history from patients • Performs examinations and assessments • Analyzes and interprets information to develop and establish appropriate diagnoses • Works in collaboration with providers and independently to provide care • Formulates a plan for treatment of the patient and prescribes medications based on efficacy, safety, and cost • Maintains complete and detailed records of patients health care plans and prognoses in a timely manner • Consults with or refers patients to appropriate specialists when conditions exceed the scope of practice or expertise • Provides patients with information needed to promote health, reduce risk factors, or prevent disease or disability • Counsels patients and family members about self-management, prevention, and treatment plans for health issues • May cover hospital consultations, rounding, and assist with patient procedures as needed • Conducts research into the area of specialty and uses findings to provide measurable improvements in patient care and clinical outcomes • Incorporates evidence-based practice guidelines into care • Maintains current knowledge of state legal regulations for advanced practice provider practices, including reimbursement of services • Keeps abreast of regulatory processes and payer systems, such as Medicare, Medicaid, managed care, and private sources, as applicable. • Clinic coverage for primary care patients in family medicine • Make up of team: solo team, new clinic, but support for neighboring Lidgerwood clinic • 2 APPs and Wahpeton Clinic • 5 MDs • 9 APPs in primary care • New Clinic location looking to have PTOT embedded in clinic as well as outreach from psychiatry services.
