The successful candidate will conduct comprehensive patient assessments, develop and manage treatment plans, coordinate care with interdisciplinary teams and promote safe, effective continuity of care. Provide education and support to patients, families and facility staff regarding diagnoses, treatment plans, medications, symptom management and preventive care.
