NewRN - Stroke Program Coordinator Albany Medical CenterRN - Stroke Program CoordinatorEast Meadow, NYMonitors delivery of care by completing patient rounds, documenting care, identifying progress towards desired care outcomes; intervening to overcome deviations in the expected plan of care, reviewing the care plan with the patient/family in conjunction with the direct care providers and interacting with involved departments to negotiate and expedite scheduling and completion of test procedures and consults, preventing care delays. • Oversees patient care by acting as a liaison to the patient care team, participating in interdisciplinary rounds and other meetings; coordinating information and care requirements with other care providers, resolving issues/pathway variances that could affect smooth care progression, fostering peer support and providing education to the multidisciplinary team regarding the hospitalization process.
