The Home Care Department Coordinator assists with home care coordination, referrals and scheduling, communicates effectively with the Nurse Manger, Home Care Nurses and Home Care Aides and builds trust and fosters smooth relationships with Clients and their families as well as Home Care Staff.
Social Service functions include but are not limited to: supervising social service staff, direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development and community relations. The Assistant Director of Social Services will abide by and practice according to the company's Social Service Policies and Procedures Manual, State and Federal guidelines and the National Association of Social Workers' "Code of Ethics".
