The Advanced Practice Clinician in the Care Anywhere (CAW) program is designed to provide care and support to our Alignment Healthcare patients by providing an additional level of medical and social support in the comfort of their own home. This program is offered to eligible patients at no cost to them with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible.
p>Essential Responsibilities: • Collect subjective data (obtain history) • Collect objective data (perform a physical exam) • Develop an assessment (diagnosis) • Develop and implement a management plan • Facilitate the referral of patients to the appropriate health facilities, agencies and resources within the community • Order pharmacologic interventions • Develop and implement an educational plan • Plan for appropriate follow-up care • Manages the care of patients with complicated medical or surgical conditions. Function in an expanded role to regularly provide primary, acute or emergency care to patients with complicated medical or surgical conditions which requires an intensive level of care and advanced certification in a specialty area (e.g., neonatal, intensive care, emergency medicine, operating room, etc.).

The Medical Assistant II & III are distinguished from Medical Assistant I in their ability to efficiently perform the job responsibilities of a Medical Assistant I and by additional medical skills, knowledge and capabilities as follows: Under the general direction and supervision of the Charge Nurse, Licensed Vocational Nurse or Lead MA the Medical Assistant II & III must possess excellent nursing skills, as well as documentation abilities in EHR system. Conducts thorough physical exams, obtains medical histories, and orders/interprets diagnostic tests;
Provides education and counseling to patients and their families on health maintenance, disease prevention, and healthy lifestyle choices;
Provides specialty care in the areas of pediatric diabetes, pediatric endocrinology and/or obesity medicine;
Collaborates with physicians, specialists, and other healthcare providers to ensure coordinated and comprehensive care;
Works directly with pediatric diabetes and endocrinology specialty along with primary care pediatricians;
Maintains accurate and up-to-date patient records in the electronic health record (EHR) system;
Participates in quality improvement initiatives and contributes to a positive patient experience; and,
Performs related duties as required.
Working ability to: perform a thorough physical examination and to determine deviations from normal; take a complete medical history, perform medical evaluations, and prescribe appropriate measures; clinically manage selected cases; maintain confidentiality of patient/client information; provide health education and counseling to patients; prepare clear and concise medical reports; work collaboratively in a multidisciplinary team; and communicate effectively, both orally and in writing.
p>Duties may include, but are not limited to the following: - Provides public health nursing services in a variety of settings, such as homes, clinics, programs, and the community;
- Performs health assessments and intervention plans for population groups and geographical areas;
- Identifies and evaluates both systems and individuals, medical, psychosocial, and socio-economic barriers to optimal health and well-being and takes the appropriate steps to mitigate their impact;
- Collaborates with other agencies and community-based organizations on behalf of individual, family, and community health promotion;
- Recognizes multicultural practices and beliefs, and their impact on health behaviors;
- Develops, implements, and evaluates nursing care plans based on identified individual, family or community needs, strengths, and availability of resources;
- Refers individuals and families to community partners based on identified bio-psychosocial needs;
- Responds to public health emergencies and disasters as needed, utilizing the nursing skills authorized under their professional scope of practice;
- Travels to client homes, community events, and various work locations; and.
LATERAL TRANSFER OPTION: If presently permanently employed in another "merit" or "civil service" public agency/entity in the same or substantively similar position as is advertised, and if appointed to that position by successful performance in a "merit" or "civil service" style examination, then appointment by "Lateral Transfer" may be possible.
