Additional Locations: Job Information: Job Summary: The role of the Specialty Care Transport (SCT) Nurse is to provide the required care during transport between designated facilities by assessing, planning, implementing, and evaluating the patient's needs during each encounter. That means bringing life-changing resources and health services directly into our communities through our Eat Well food access program, telehealth, home health, rehabilitation, mobile screenings, paramedic programs, and convenient online scheduling.
