Core Responsibilities and Essential Functions: DIRECT PATIENT CARE* Assesses, coordinates with other disciplines, and manages patients total needs, following physicians orders, using the nursing process.* Develops patients plan of care coordinating with other disciplines.* Performs routine and complex nursing procedures to meet the need of the patient. The RN Home Health is responsible for the coordination of initial disciplines to begin care, including the planning of care throughout the patient episode, collaborating with the physician, patient, family member and team manager to develop the expected patient visits, scheduling the visits to the appropriate team member, incorporating therapeutic, preventative and rehabilitative nursing actions.
