Provide support, instruction, and education of the patient, family and other caregivers who participate in the care of the patient in relation to needs identified on the plan of care, including, but not limited to: disease process and progression, medications, pain, symptoms associated with disease, oxygen safety, hospice philosophy, and care of the terminally ill. Managing all aspects of the patient’s plan of care, based on the changing needs of the patient and family/caregiver, to anticipate, prevent, treat, and manage pain and other undesirable symptoms through ongoing communication of collected data and assessment findings, and collaborating with other interdisciplinary team members.
