This role is ideal for an experienced Registered Nurse, RN Supervisor, Pediatric Nurse Manager, Clinical Nurse Manager, Ambulatory Nurse Manager, or Lead RN who enjoys improving clinical workflows, developing nursing staff, supporting patient-centered care, and leading a multidisciplinary team. Manage caseload of (4) terminally ill Patients per day, generate and follow plan of care to provide patients with Maximum level of comfort and quality of life in home, Skill nursing facilities and In-patient units settings Coordinate care efforts among LPN, HHA and care givers. Educate Patients, Families and Care givers on medical choices and methodologies Assess patients weekly and as needed to identify necessity for plan of care revisions and seek Interdisciplinary expertise to coordinate highest level of care Gather information on patients and families psychosocial and spiritual unit for symptoms management .
