Located in Montclair, NJ, the 365 bed medical center provides patients immediate access to innovative and effective treatment alternatives at specialized centers within the medical center that focus on imaging, women's health, cancer care, surgery, obesity, stroke and chronic kidney disease. We are searching for a compassionate caregiver who will assist in guiding patients through the cancer care journey- connecting them with internal/external resources by assisting with coordinating multidisciplinary care and ensuring timely referrals and treatment initiation.

In it, you will conduct clinical assessments, monitor for changes in health, coordinate care, including transitions, and educate members and caregivers about their diagnosis and treatment over the phone to support our members as they move through the oncology care continuum.

With 554 beds, including facilities for long-term care and behavioral health, Trinitas annually treats nearly 20,000 inpatients, 70,000 emergency patients and accommodates over 450,000 outpatient visits.

li>One year prior experience as a Registered Nurse in an Oncology unit in-patient or outpatient strongly preferred, Certifications and Licenses Required: Responsibilities include patient assessment, care coordination, symptom management, patient education, treatment planning support, triage, and collaboration with physicians and the multidisciplinary care team to ensure safe, high-quality care throughout the patient's cancer journey.
p>The Hematology/Oncology Ambulatory Care RN provides compassionate, patient-centered nursing care to oncology and hematology patients in an outpatient clinic setting, Responsibilities include patient assessment, care coordination, symptom management, patient education, treatment planning support, triage, and collaboration with physicians and the multidisciplinary care team to ensure safe, high-quality care throughout the patient's cancer journey.
