This position completes health risk assessments, conducts patient outreach, schedules follow-up appointments for patients recently discharged from the emergency department or inpatient care, and provides ongoing education related to preventive services and appropriate healthcare utilization. As a Care Coordination Outreach Worker, you'll help patients stay connected to the healthcare services they need by completing health risk screenings, coordinating follow-up care, providing education, and conducting outreach in clinic, hospital, and community settings.
