The Outreach Worker will regularly visit and work with individual clients to identify and overcome barriers to housing and healthcare and refer these households to other resources in the community (mainstream services, benefit services, health care, food assistance programs, childcare resources, etc.). License and Certifications: Knowledge, Skills and Abilities Required: Strong Human relations skills; trauma-informed care; community building, and motivational interviewing skills are preferred, (but will train the right person).
