Working closely with healthcare providers, behavioral health specialists, case managers, social service agencies, and community partners, the Community Paramedic helps patients navigate healthcare and community resources while improving access to care, reducing avoidable emergency department visits and hospital readmissions, and decreasing inappropriate utilization of 911 services. The Community Paramedic is a key member of an interdisciplinary healthcare team providing patient-centered care in homes, community settings, and other non-traditional care environments throughout central and northeast Vermont.
