At least 2 years of experience working within hospital, post-acute, or physician practice environments with knowledge of healthcare workflows, patient transitions, and provider operations with proven track record of building successful relationships with physicians, hospital staff, SNF partners, and community providers. This role is responsible for coordinating daily provider operations, managing patient tracking and assignments, supporting transitions of care, facilitating communication across the continuum, and ensuring accurate documentation, billing, payroll submission, and scheduling activities.
