Makes direct/indirect contacts in accordance to the person-centered plan with the assigned individuals, and is responsible for assuring the implementation, coordination, and monitoring of the person-centered plan of service in residential, employment, school, and other settings. For individuals requiring specialized, contracted, and other placement services, the Clinician completes and coordinates a residential treatment plan and visits the home at least monthly to monitor progress in the placement setting.
