The Director for Community Prevention and Treatment Services (CPTS) Program(s) is responsible for oversight, implementation, and continuous program improvements for HIV/AIDS, Hepatitis C primary medical care, medical case management, behavioral health integration, prevention, outreach and support services-designed to improve health outcomes for people living with or at-risk for HIV/AIDS, Hepatitis C, and/or other Sexually Transmitted Infections (STI). · Provides oversight and facilitates the multidisciplinary case conference consisting of multidisciplinary team members including physicians, nurse practitioners, physician assistants, nurses, medical case managers, behavioral health clinicians and SUD counselors, outreach staff, and peer support providers.
