Home Based Duties: . Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities.
We are seeking a Home Care Personal / Care Aide to join our team. The ideal candidate is patient, compassionate, and reliable.
Responsibilities
- Assist with personal hygiene needs and dressing.
I authorize Abernathy Home Care, Inc. and its designated representatives to contact my current and former employers, supervisors, professional references, educational institutions, credentialing organizations, and other relevant parties for the purpose of verifying information related to my application for employment, including but not limited to employment history, job performance, qualifications, professional conduct, licensure, certifications, and eligibility for employment.
I understand that this authorization includes communication by phone, email, fax, or other lawful means for employment verification and reference checking purposes.
I authorize all individuals, employers, organizations, and entities contacted by Abernathy Home Care to release information concerning my employment, qualifications, and professional background, to the extent permitted by law.
I understand that Abernathy Home Care will use this information solely for employment-related purposes in connection with evaluating my candidacy for employment.
I release Abernathy Home Care, Inc., and, to the extent permitted by law, any person or entity providing such information, from liability arising out of the furnishing or use of this information in good faith for employment purposes.
I acknowledge that I have read, understand, and voluntarily agree to this authorization.
