Effectively coordinate the scheduling of home care personal care assignments as directed by the Home Care Supervisor, including adjusting schedules based on changes and communicating promptly with all stakeholders (patients, families, home services staff, etc.). Working under the guidance of the Home Care Supervisor, you will ensure our patients receive timely, high-quality care at home while managing schedule adjustments, processing EMR authorizations, and verifying care logs.
