Position description: The oncology/hematology infusion pharmacist is responsible for drug distribution, drug therapy monitoring, patient education, chemotherapy order processing, product verification, precepting learners/residents, participating in research projects, leading and participating in quality improvement initiatives, and providing integrated care and medication therapy management services to patients receiving treatment. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services.
