Duties include reviewing prescriptions and medication orders for accuracy, appropriateness, and completeness; monitoring patient drug therapy; identifying clinically significant drug interactions, adverse reactions, therapeutic duplications, and dosage concerns; and ensuring compliance with prescribed treatment plans. Additional responsibilities include recommending therapeutic alternatives, managing and expanding the medication formulary, participating in interdisciplinary healthcare initiatives, and providing medication-related education and presentations to executive leadership and clinical staff.
