In collaboration with the Hospice physicians and nurses, performs ongoing assessment of patient risk factors, indications for therapy and works for optimization of therapeutic regimens, adjusting medication therapy as described in protocol and as authorized by CDTA. Commits to instructing, modeling and coaching pharmacists, technicians, assistants, students, residents and other health care disciplines as deemed appropriately by the Pharmacy Director.
