Serves as a Liaison between patients, caregivers, home health agency nurses, hospice nurses, and the prescribing physician/clinician for orders related to the services provided by Biologics, Inc. to ensure all stakeholders receive outrageous customer service. Coordinates patient care with the referring clinician, including but not limited to, social, psychological and physical needs that warrant customized services by Biologics, Inc. (i.e. pre-filled syringes, color coded labeling for medication identification, etc.) and documenting these services in their progress note.
