Responsibilities: Coordinates delivery of medications in a timely manner to patient''s preferred location (bedside, home, outpatientpharmacy) to ensure patients receive treatment in a timely mannerCollaborates with clinical staff to facilitate patient and/or caregiver requestsPerforms patient intake, benefits investigation, obtains copay assistance and provides link to patient assistance or triagesto other pharmacies or servicesPerforms activities related to pharmacy technician scope of practiceMaintains compliance with department workflow assignments and metricsCommunicates effectively in person, over the phone and in writing to provide customer service to the internal and externalcustomers (patient, caregiver, physician, pharmacist) and liaisons between patient and providers for medication needs,including patient information, prescription processing, third party rejection resolution, delivery, storage, supplies, refills,education materialsCoordinates care with clinical staff with any patient side effect and/or request from providers, patient or caregiverSupports staff training, transitioning and onboardingCollects, documents and reviews all needed information to ensure timely coordination of care.
