Provides education to patients, family members, and/or caretakers about disease states and medications, including: • Desired results and/or goals of chronic disease states • Name, strength, description, and purpose of each medication • How to administer each medication timely, safely, and appropriately • How agents work to attain adequate therapy • Interacting foods, disease states, and drugs where appropriate • Importance of therapeutic drug monitoring for efficacy and toxicity • Recognition of disease state exacerbation and procedures to follow in these instances • Recognition of side effects of therapy and the importance of notifying his/her physician if any should occur • Importance of adherence to pharmacotherapy and appointments • Aids to increase medication adherence. Educational/Professional Development • Participates in the development of other staff members by identifying the learning needs of pharmacy staff and other healthcare professionals through education on but not limited to: clinical appropriateness of medications, dose adjustments, drug selection based on formulary, drug interactions, drug cost, and patient safety • Serves as a preceptor for the residency program, and for students from local colleges of pharmacy • Designs and implements appropriate trainings and development programs in a timely fashion to maintain contemporary practice.
