The Clinical Pharmacy Specialist serves as a subject matter expert in clinical pharmacy practice and provides comprehensive medication management services for behavioral health conditions, substance use disorders, diabetes, metabolic disorders, ambulatory care, infectious disease prevention and treatment, population health, and other chronic disease states consistent with organizational programs, practitioner competency, and scope of practice. The Clinical Pharmacy Specialist provides advanced, patient-centered clinical pharmacy services with a focus on medication optimization, disease state management, specialty pharmacy practice, and interdisciplinary collaboration.
