JOB SUMMARY: The Clinician in a Post-Acute Care setting provides comprehensive evaluation, treatment, and care management services to patients transitioning from acute hospitalization to rehabilitation, skilled nursing, long-term care, or home-based care environments. SCOPE OF SERVICE: The BMH-owned or managed physician practices provide medical services in various specialties and offices including pediatrics, OB/GYN, cardiology, orthopedics, general surgery, family practice and internal medicine.
