Responsibilities: The Physician Assistant Provider will work in a culturally diverse environment with patients and families of any background and will be collaborating with Physicians, Nurse Practitioners, and IDT in providing palliative care consults with patients regarding pain and symptom management, advance care planning, and hospice eligibility evaluations. The PA will self-schedule all patient visits and will independently conduct comprehensive patient assessments, treatment planning, and follow-up care, ensuring care plans minimize illness burden and align with patients’ goals and values.
