Minimum of one year of clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred. Providers will work collaboratively with interdisciplinary teams to manage chronic and acute medical conditions, support recovery, reduce hospital readmissions, and optimize patient outcomes in the post-acute setting.
