This role serves as a part of the clinical quality leadership team and collaborates with clinical operations, physicians and APPs, care teams, payor partners, UCSF population health teams, MarinHealth Medical Center partners, and population health programs to lead initiatives ensuring patients receive timely, evidence-based care while meeting organizational and contractual quality goals. Under the direction of the Director of Ambulatory Quality, and guidance of the Medical Director of quality and Ambulatory operational leadership team, the Population Health Program Manager contributes to the development and execution of strategies to improve preventive care, chronic disease management, risk adjustment performance, clinical outcomes for managed care patients, care coordination, and patient engagement.
