The Program Manager serves as the primary operational and implementation lead responsible for overseeing the development, coordination, execution, and continuous improvement of the Southern Delaware Food Is Medicine (SDFIM) infrastructure initiative, including workforce deployment, technology integration, culinary medicine capacity building, community-based interventions, care coordination systems, evaluation readiness, partnership management, and grant compliance. Coordinate Epic workflow design, patient identification processes, referral management systems, and operational activities collaborating with clinical, operational, and technology teams to ensure seamless integration across clinicians, Registered Dietitians, Community Health Workers, nutrition education services, and partner systems.
