Assists patients/families with services including but not limited to primary care, specialty care visits, skilled homecare, palliative care, hospice care (including hospice residence), and/or skilled nursing facility, to ensure appropriateness of services and expedite transitions of care. The triage nurse duties include but are not limited to: Providing telephone triage and clinical advice appropriately; coordinating follow-up care of patients, providing patient education, case management support, and practice management support.
