Crossroads Health, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one's ability to pay. The Integrated Care Registered Nurse (RN) offers support for our prescribers, delegation, and support for medical assistants, direction, and support to Licensed Practical Nurses (LPN) and provides integrated primary care and behavioral health nursing services for patients (across the life span) within the Health Center Program.
