Maintain organized files and document all interactions in the Customer Relationship Management tool ("CRM")Support recruitment and contracting efforts by developing strong interpersonal relationships with providers, hospitals and ancillary providersPromote access to care by connecting rural community members to healthcare services, benefits, programs, and resources, ensuring they are informed about available options that address their unique health needsConduct outreach activities, including presentations and providing educational materials, to increase awareness about health issues and promote healthy behaviorsCollect and analyze data and insights on specific health needs and challenges faced by rural communities to identify outreach opportunitiesProvide input for policy development and support the implementation of innovative solutions to the meet the unique needs of rural communitiesFoster, coordinate, and facilitate cultural competency training for CareSource staffProvide support and resources to providers and CBOs to enhance efforts and address gaps in care and health literacyOrganize and host community townhalls, rural member/provider/community advisory groups, listening sessions and other events to inform, educate, gather data and informationEstablish relationships among rural community-based organizations, stakeholders and providers to stay abreast of rural health challenges and opportunitiesCollaborate with Managed Care Organization (MCO) to address social determinants of health and integrating wellness and holistic healthPromote MCO plan services and benefits to improve the health equity in rural communitiesDevelop comprehensive success metrics that demonstrate the impact and value of Rural Health initiatives and outreach to members, providers, and CBOsPerforms other job related instructions as requestedEducation and Experience:Bachelor's degree or equivalent years of relevant work experience is required, preferably in social work or public healthMinimum of three (3) years of health care, public health or community based organization experience or experience related to the responsibilities of the position is requiredFamiliarity of rural health communities, rural health disparities and knowledge of Medicaid preferredCompetencies, Knowledge and Skills:Proficient in Microsoft Office especially, Word, PowerPoint, Excel and AccessFluency (speaking, reading, and writing) in multiple languages including English and Spanish is preferredSensitivity to and experience working within different culturesAbility to work independently and in a team environmentKnowledge of community and state support resourcesAbility to work with difficult situations with multiple interests/parties involvedStrong planning, organizational, multi-tasking skillsStrong interpersonal skills, including active listening and relationship/trust/consensus buildingResearch and analytical skillsDemonstrated program development skillsExcellent written, verbal and presentation skillsSpanish speaking preferredLicensure and Certification:Community Health Worker certification is preferredMust have valid driver's license, vehicle and verifiable insurance. Essential Functions:Engage with members, providers, and community-based organizations (CBOs), and other stakeholders to share information, resources, and provide education on health access and servicesProvide proactive, high-level account development and support in order to drive the overall satisfaction and engagement of key rural health partnersHandle, refer, or delegate complex or non-routine questions, issues, and problems to appropriate resources as they relate to the assigned accounts and ensure they are resolved in a timely manner.
