Position Summary The PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards. Education, Experience, and Licensure Requirements <ul><li> Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)</li><li> Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment. </li><li> One (1) year experience in post acute care or related setting preferred </li><li> CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment. </li></ul> Specific Job Requirements <ul><li> Participates in community health matters/projects as appropriate </li><li> Proficient in Microsoft Word, Excel, and e mail </li><li> Liaisons with patients, families, support departments, etc., to adequately plan for patient needs </li><li> Must demonstrate good body mechanics at all times </li><li> Make independent decisions when circumstances warrant such action </li><li> Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility </li><li> Implement and interpret the programs, goals, objectives, policies, and procedures of the department </li><li> Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation </li><li> Maintains professional working relationships with all associates, vendors, etc. </li><li> Maintains confidentiality of all proprietary and/or confidential information </li><li> Understand and follow company policies including harassment and compliance procedures </li><li> Displays integrity and professionalism by adhering to Life Care’s Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training </li></ul> Essential Functions <ul><li> Follow physical therapy treatment plans for patients </li><li> Establish, assess, and modify realistic, measurable, timely, and functional goals </li><li> Oversee and evaluate care given by PTAs, Rehab Aides, and PT and PTA students </li><li> Chart appropriately and timely </li><li> Utilize therapy software appropriately and accurately </li><li> Exhibit excellent customer service and a positive attitude towards patients </li><li> Assist in the evacuation of patients </li><li> Demonstrate dependable, regular attendance </li><li> Concentrate and use reasoning skills and good judgment </li><li> Communicate and function productively on an interdisciplinary team </li><li> Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours </li><li> Read, write, speak, and understand the English language </li></ul> An Equal Opportunity Employer

The DOR is delegated the administrative authority, responsibility, and accountability necessary for carrying out all assigned duties to provide both clinical and operational oversight to the delivery of rehabilitative services.

General PurposeThe Director of Rehabilitation (DOR) is responsible for the day-to-day activities in the rehabilitation department of an assigned facility or facilities. This includes but is not limited to the delivery of direct and indirect services to in-patients and out-patients, supervision of all therapy personnel, interviewing and recommending new hires, and training of all therapy personnel.The DOR is responsible for the implementation of new programs to meet the needs of the facility and the residents. The DOR is delegated the administrative authority, responsibility, and accountability necessary for carrying out all assigned duties to provide both clinical and operational oversight to the delivery of rehabilitative services.Essential Duties <ul><li>Directly supervises and guides all therapy personnel in an assigned facility, including licensed, registered, certified therapists, assistants, aides, and students.</li><li>Performs daily scheduling for department staff to meet the needs of the facility.</li><li>Provides direct care and works with supervisor to determine appropriate caseload in tandem with management responsibilities.</li><li>Assures that the rehabilitation needs of the residents are met.</li><li>Periodically reviews therapy services offered and updates them to meet the changing environment.</li><li>Ensures adherence to all regulatory standards as they pertain to the delivery of rehabilitation services.</li><li>Ensures that all clinical documentation is complete, timely, and accurately reflects the services provided.</li><li>Conducts periodic department documentation reviews.</li><li>Coordinates denials management activities of the department to ensure that claims and appeals are submitted timely and in accordance with company procedures.</li><li>Meets and/or exceeds productivity standards as established with the supervisor.</li><li>Ensures that the services delivered are of the highest quality and meet all standards of practice.</li><li>Implements new treatment programs.</li><li>Monitors treatment programs for outcomes and adjusts plans of care as necessary.</li><li>Ensures the department follows established policies and procedures of the facility and the company.</li><li>Approves timecards and verifies time worked by department personnel.</li><li>Provides evaluation input on department employees and builds relationships with local hospitals and referral sites.</li><li>Attends and participates in facility and company meetings as agreed to with facility leadership and approved by the supervisor.</li><li>Consistently in-services all staff regarding safe work practices, emphasizing proper body mechanics, safety, and judgment to minimize work-related injuries.</li><li>Organizes rehabilitation orientation for new department personnel, ensuring all paperwork is completed timely and distributed to appropriate human resources personnel.</li><li>Completes billing documentation timely and accurately, reflecting services provided in accordance with company standards.</li><li>Provides recommendations to supervisor on opportunities to improve company operations and services provided.</li><li>Must be able to travel, whether by car and/or plane, as necessary to reach assigned destinations in a reasonable time frame.</li><li>Must maintain confidentiality of patient and company information.</li><li>Identifies own areas of need for development, training, and continuing education in conjunction with supervisor.</li><li>Identifies potential leaders in the company as matched with specific job requirements.</li><li>Identifies new sources of business and contacts.</li><li>Promotes a positive attitude to staff, within staff, and to the community regarding the company, its mission statement, philosophies, and goals.</li></ul>Supervisory RequirementsSupervises PT, OT, and Speech Therapy therapists, assistants, aides, and students.QualificationEducation and/or ExperienceBachelor's degree in Physical Therapy, Occupational Therapy, and/or Speech Language Pathology required.Master's or Doctorate degree in Physical Therapy, Occupational Therapy, and/or Speech Language Pathology preferred.Proficient in computer skills, including inputting data into programs such as Casamba and PointClickCare.Language SkillsAbility to read technical procedures.Ability to read and comprehend policy and procedure manuals.Ability to effectively present information and respond to questions from managers, coworkers, and families.Mathematical SkillsAbility to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.Reasoning AbilityAbility to solve practical problems.Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.Certificates, Licenses, RegistrationsLicensed as a Physical Therapist, Occupational Therapist, and/or Speech Language Pathologist in the state.Employee must meet continuing education requirements per state practices.Must maintain a license in good standing at all times with the state board.CPR certification required.Physical DemandsThe essential functions of this position require the following physical abilities: <ul><li>Standing and/or walking very frequently.</li><li>Sitting occasionally.</li><li>Reaching with hands and arms frequently;
