Collaborate with an interdisciplinary team that includes physical therapists, occupational therapists, speech-language pathologists, social workers, CNAs, and fellow RNs to help patients achieve their rehabilitation goals. Teach patients and families the skills, strategies, and self-care techniques needed to support recovery and maximize independence after discharge.
