ESSENTIAL JOB DUTIES: Assists in performing evaluations and periodic re- evaluations of residents describing the level of psychosocial impairment, physical health problems, self-care potential, support network availability, adequacy of living arrangements, financial status, employment status and potential training or educational needs. Monitors, supports and assists the resident in developing or maintaining skills needed to achieve the Mental Health Treatment Plan, including medical, dental and psychiatric services, guidance in money management, and the use of educational, socialization, rehabilitation and other social services.
