The CCMHC is responsible for providing triage, clinical assessment, direct diagnosis of mental health disorders in conjunction with DSM, ICD-10 and Collaborative Care and/or Psychotherapy codes, psychoeducation to patients and primary care providers, care coordination, case management, direct evidence-based psychotherapy, treatment plan formulation and updates, crisis coordination, and relapse prevention planning. STRONGLY PREFERRED: Experience working as part of a multidisciplinary team (including experience with integrated behavioral health care models including Collaborative Care).
