JOB SUMMARY: Under the direction of a designated supervisor/manager, the Community Health Worker assists with the development and ongoing continuity of collaborative relationships with community sites and programs for designated health center services and programs. Provides community health outreach and other support services to targeted populations to improve awareness about designated program(s) and resources (e.g., medical, oral health, mental health, prenatal, family, homeless, migratory programs, HIV/AIDS, etc.).
