You will only see FOUR Hospice patients per day and will manage caseload of terminally ill, end-of-life patients, delivering skilled nursing care in home and facility settings, supporting patients and families throughout the end-of-life process. Collaborate with Physicians to regularly titrate medications and update individualized care plans while simultaneously working with IDT team in developing comprehensive, patient-centered plans of care.
