Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the department's applicable services which may include:• Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up)• Total parenteral nutrition (TPN) consults• Anticoagulation initiation and follow-up pursuant to a consult• Renal dose adjustments• Intravenous to oral conversions• Additional clinical opportunities may be available based upon departmental needs, clinical experience, and performance, and may include unit-based pharmacist, antimicrobial stewardship, patient care rounds, or pain management team• Direct patient care as part of interdisciplinary team, if applicable, through development of individualized treatment and monitoring plans for patients• Assists with medication reconciliation during all changes in level of care, including but not limited to, transfers and postoperative care. Provides pharmacy management and oversight of: • Integrity and security of medications in all care locations • Investigational drugs• Preparation of injectable chemotherapy• Medication wastage, returns, and recalls• Reconciliation of controlled substance wastage• Compounding of formulations• Automated dispensing cabinets• Medication label components and label quality• Procurement and inventory management• Quality control assessment• Coaches and mentors pharmacy technicians and Level I through Level IV pharmacists while staffing in the distributive domain• Pharmacy operations as a pharmacist-in-charge (if applicable)• Validates regulatory compliance.
