Provide individual case management/file documentation for assigned caseload, noting all treatment related activities and services in an accurate, professional, and timely manner including assessment of substance use and mental health problems, creating and monitoring individualized treatment plans, providing group therapy and individual counseling services to individuals with co-occurring substance abuse and mental health issues. The Counselor provides services to support achievement of effective personal development working with clients individually and in group settings to identify and resolve issues including substance use, mental health, personal and/or emotional and family issues.
