As a full‑time Nurse Practitioner, you will collaborate with physicians, RNs, case managers, and interdisciplinary care teams to diagnose, manage, and prevent acute and chronic conditions while promoting wellness and quality of life. The Nurse Practitioner (NP) provides holistic, compassionate, and patient‑centered care to adult and geriatric Members within skilled nursing and long‑term care settings.
