Acts as Liaison for patients, families, and their multidisciplinary clinical team to address the population health needs of our patients by assessing for clinical, emotional, spiritual, psychosocial, and financial barriers to care; Once identified, works with care team to find a plan to overcome barriers for successful health outcomes. The Journeys and Adventures that Await: As a dedicated care navigator, the Care Ally RN demonstrates proactive and ongoing guidance, provides a bridge of understanding between the clinical and the patient worlds, and in so doing helps patients set, keep, and achieve their health goals.
