Acts as Liaison for patients, families, and their multidisciplinary clinical team to address the population health needs of our patients by assessing for clinical, emotional, spiritual, psychosocial, and financial barriers to care; Once identified, works with care team to find a plan to overcome barriers for successful health outcomes.
