The Registered Nurse under the direction and supervision of the Clinic Manager, provides skilled nursing care to participants in the PACE clinic and in participant’s homes, implements primary care orders, evaluates participant symptoms in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team. With the internal slogan "The care you need to remain in the home you LOVE", this "one stop shop" prides itself in building relationships with the participants as well as their families, and creating personalized care plans that work for everyone.
