Required Qualifications Minimum 2 years of acute-care hospital Case Management experience Recent acute medical Care Manager experience performing care coordination within the last 3 months Previous travel experience required Epic experience required Active California RN license required by the start date; pending license may be accepted Experience with discharge planning and independent care coordination Experience working with HMOs, IPAs, and managed-care organizations Ability to independently arrange DME, skilled nursing facility (SNF) placement, and ambulance transportation Knowledge of advance directives, POLST, and protective services processes.
