This is an excellent opportunity for a detail-oriented healthcare professional who enjoys organization, collaboration, and making a meaningful impact on patient access to care.Duties<ul class="list-disc pl-8"><li>Generate specialty referrals and schedule both in-house and external medical appointments.</li><li>Prepare and provide all required documentation for medical and specialty appointments.</li><li>Cancel or reschedule appointments as needed and notify patients and families promptly.</li><li>Enter, track, and maintain all in-house and external appointments within the scheduling system.</li><li>Complete and manage paperwork related to all medical appointments.</li><li>Schedule and coordinate audiology, podiatry, dental, optometry, and other specialty referrals.</li><li>Generate referral forms for visiting Primary Care Physicians (PCPs) and enter them into the scheduling system.</li><li>Send completed appointment documentation to PCPs following patient visits.</li><li>Obtain, track, and manage diagnostic requests and billing forms as required.</li><li>Prepare podiatry related forms for podiatry visits.</li><li>Communicate clearly and professionally—both verbally and in writingw ith providers, patients, families, and team members.</li><li>Update the scheduling system promptly to reflect appointment changes.</li><li>Prepare and maintain monthly specialist calendars within practice management software.</li><li>Provide coverage for other schedulers as needed.</li><li>Contact facilities to obtain test results when not received in a timely manner.</li><li>Prepare appointment-related paperwork for meals and transportation coordination.</li><li>Coordinate appointment-related services including transportation, meals, translation, and patient escorts.</li><li>Perform all other reasonably related duties as assigned.</li></ul>Required Qualifications<ul class="list-disc pl-8"><li>Minimum of two (2) years of experience in a healthcare setting; experience working with elderly populations preferred.</li><li>Knowledge of healthcare operations with demonstrated experience in medical scheduling and billing across diverse patient populations.</li><li>Excellent written and verbal communication skills.</li><li>Proficiency with computers and healthcare scheduling or practice management systems.</li><li>Flexible, proactive, and willing to support all aspects of organizational operations.</li><li>Bilingual In Spanish and English.</li></ul>Desired Qualifications<ul class="list-disc pl-8"><li>Experience working in a multi-specialty or community-based healthcare environment.</li><li>Strong organizational skills with the ability to manage multiple priorities effectively.</li><li>Experience coordinating ancillary services such as transportation, meals, or translation support.</li></ul>Location: On-site

This position assists the Assisted Living Director with overseeing aspects of resident care, supervises assisted living and memory care services, maintains medical supplies inventory, and implements health, orientation, and training programs for associates and residents. This position also requires 2-3 years of supervisory experience, prior care experience in an assisted living community or skilled nursing facility, and valid First Aid and CPR Certification.

Perform activities on schedule while maintaining regular attendance and punctuality within specified tolerance.</li><li>Maintain clear records of all resident changes, requests, and irregularities in coordination with Assisted Living Director.</li><li>Update care plans in coordination and with direction with the Assisted Living Director.</li><li>Record and notify Assisted Living Director, Executive Director, physician, pharmacist, and family of any significant changes to resident’s health status and/or reactions to drugs, treatments, or significant incidents.</li><li>Assist as needed with order medications from pharmacy, maintain records, and ensure that medications are properly stored and cared for.</li><li>Audit med room and charts weekly.</li><li>Review and implement incident reports; evaluate for need of intervention to minimize risk of further occurrences. Escalate incidents to nurse for review.</li><li>Read and understand medications and charts.</li><li>Maintain records and files for residents, inventory, and supplies.</li><li>Maintain appropriate supplies at all time.</li><li>Provide a quality assisted living and memory care program.</li><li>Monitor associates in administration of mediation, including frequent audits of medication records.</li><li>In coordination continually assess resident needs in relation to staff training needs to determine priorities in designing an ongoing orientation program.</li><li>Participate in New Hire Orientation program and other regular staff meetings or functions.</li><li>Report concerns and problems to supervisor.</li><li>Support staff and leadership in On-Call responsibilities</li><li>This position also requires 2-3 years of supervisory experience, prior care experience in an assisted living community or skilled nursing facility, and valid First Aid and CPR Certification.</li><li>This position requires 2-3 years of experience with medication passing and management.</li><li>Able to speak, read, write and understand the English language fluently.</li><li>Must have reliable transportation to travel for assessments and other outreach, automobile insurance as required by law and a valid driver’s license.</li></ul>We look forward to you joining our team!
